12-2875 Ratification by the Board of Education of a Professional Services Contract between the District and Rochelle Weintraub, Oakland, CA, for the latter to provide 235 hours of support to teachers with the BTSA Induction program requirements and to mentor teachers in classroom practices; meet with the new teachers assigned to him/her during the school year; guide new teachers in improving their classroom practices and ensure that teachers complete the requirements for their Clear Credential, for the period of October 1, 2012 through June 29, 2012 in an amount not to exceed $10,000.00.

12-2902 Ratification by the Board of Education of a Professional Services Contract between District and Clarence Holmes, Oakland, CA, for the latter to provide a maximum of 575.00 hours of service as follows; 1) implement Accelerated Reader resources and systems on campus daily; 2) train staff on best practices of utilizing Accelerated Reader three times annually with additional support as needed; 3) maintain classroom libraries and ensure equitable access to appropriate reading materials; and 4) update inventory of books for library classrooms and facilitate additional purchases as needed three times annually, at Elmhurst Community Preparatory School, for the period of August 27, 2012 through June 28, 2013, in an amount not to exceed $23,000.00.

12-2915 Ratification by the Board of Education of a Professional Services Contract between the District and Elizabeth Lindsley, Oakland, CA, for the latter to provide 250 hours of services implementing the Arts Learning Anchor School Program in 28 Oakland schools K-12 through the guidance of a team of facilitators, who will: work with the site principal to identify a teacher leader in the arts; where necessary assist site leadership in implementing their arts program; provide 30 hours of training for teacher leaders from ALAS K-12 sites throughout the district; build capacity for leadership and an understanding of quality arts education in the teacher leaders involved; support the teacher leader in developing a site-based plan for their arts education program for the 2012-13 school year; create a platform for documenting and sharing the arts education work being done at each site for all to learn from and other duties, if any, as described in the Scope of Work, incorporated herein by reference as though fully set forth, period October 12, 2012 through June 30, 2013, in an amount not to exceed $15,000.00.

12-2918 Ratification by the Board of Education of a Professional Services Contract between the District and Linda Luevano, San Francisco, CA, for the latter to provide 334 hours of services by gathering data on research based Dual Language Programs; work in collaboration with the management and site administrators to plan professional learning for site leaders and teachers; gather data on strengths and challenges of current programs in OUSD and work with site leadership to follow through the next steps; provide recommendations to support sustainable Dual Language programs across District; and support identification of resources, for the period October 15, 2012 through June 30, 2013, in an amount not to exceed $30,000.00.

12-2928 Ratification by the Board of Education of a Professional Services Contract between District and Janise Erickson, Pleasanton, CA, for the latter to provide 1556 hours of service to select sites to implement the Second Step Program, train teachers to deliver the Second Step curriculum to students, collaborate with staff to collect data on completion of Second Step lessons, and build teacher knowledge and capacity to sustain implementation of the program for the period of October 15, 2012 through June 30, 2013.

12-2936 Ratification by the Board of Education of a Professional Services Contract between District and Alexandria N. Shelling, San Pablo, CA, for the latter to provide mental health counseling services including crisis intervention; parent and teacher consultation including assisting with behavior management plans; individual and small group counseling, parent outreach and education, and case management, for the period of September 4, 2012 through June 30, 2013, in an amount not to exceed $500.00.

12-3003 Ratification by the Board of Education of the Memorandum of Understanding between the Oakland Unified School District and Stephanie Farland, a charter school consultant, for the latter to support the work related to a Notice of Violation issued to the American Indian Model School for the contract term November 1, 2012 through June 30, 2013 in an amount not to exceed $30,000 per fiscal year.

12-3112 Ratification by the Board of Education of a Professional Services Contract between District and Amy Valdez-Villagomez, Oakland, CA, for the latter to provide appropriate training and materials to assist each DELAC member with carrying out his or her legally required advisory responsibilities; training in full consultation with DELAC members along with maintaining the parent attendance tracking database; supporting DELAC parent engagement activities such as recruitment, for the period of October 11, 2012 through June 30, 2013, in an amount not to exceed $20,000.00.

12-3114 Ratification by the Board of Education of a Professional Services Contract between District and Joyce Boykin, Oakland, CA, for the latter to provide 350 hours of supplemental instructional services for eligible and identified private school students, including providing effective and organize extended support services in reading, language arts, and mathematics to identified students; provide standards-based instructional support to students; work with other school staff to present compliant annual Title I Program meeting for parents; confer with parents as needed, as described in the Scope of Work, incorporated herein by reference as though fully set forth, at St. Martin de Porres School, for the period October 15, 2012 through June 30, 2013, in an amount not to exceed $17,500.00.

12-3127 Ratification by the Board of Education of a Professional Services Contract between District and Slimgim Designs, Richmond, CA, for the latter to provide the full spectrum of visual design services; working across all mediums--print, digital, etc.;work closely with various department leads to understand their strategic and tactical needs, so that these can be effectively communicated through collateral;layout of the 2013-14 Options Brochures (and all translations), 2013-14 Parent Guides (and all translations), Back-to-School campaign materials for the 2013-14 school year, the College-Bound Graduates promotional poster, logo, letterhead, and business card design; additional projects as needed, for the period of October 1, 2012 through June 30, 2013, in an amount not to exceed $50,625.00.

12-3134 Approval by the Board of Education of a Memorandum of Understanding - Transitions Data Initiative between District and the Institute for Evidence-Based Change (IEBC), Encinitas, CA, for the latter to collect District K-12 student data and be able to link such data to community college and university data through its data sharing agreements with pariticpating postsecondary institutions, pursuant to the terms and conditions thereof, for the period of October 15, 2012 through August 31, 2017, at no cost to the District.

12-3135 Acceptance by the Board of Education of California Department of Education Grant Award to the District for the Fresh Fruit and Vegetable Program in the amount of $452,578.11, providing additional funding for the serving of fresh fruit and vegetables to students outside of existing breakfast or lunch program, including allowing Nutrition Services to expose students to a wide variety of produce, including non-traditional items like cumquats, persimmons, etc, for the period of October 1, 2012 through June 30, 2013, pursuant to the terms and conditions thereof.

12-3140 Approval by the Board of Education of a Professional Services Contract between District and Higher Ground Neighborhood Development Corporation, Oakland, CA, for the latter to provide 102.56 hours of service to increase the participation of ELL families at school events, i.e., Data Night, Family Literacy Night, ELAC and SSC meetings, focusing on increasing parents and caregivers access to resources to support their children's education; provide weekly ongoing translations for non-bilingual teachers by writing notes and making phone calls to Spanish-speaking parents regarding school events, student progress, challenges and successes; and work to organize parent volunteers, for the period October 28, 2012 through June 14, 2013 at new Highland Academy, at a cost not to exceed $1600.00.
