13-2696 Ratification by the Board of Education of a Professional Services Contract between the District and La Clinica de La Raza, Inc., Oakland, CA, for the latter to 1) collect completed consent forms from designated school nurses; 2) follow up with families around missing information; 3) provide follow up eye exams at elementary and middle school sites; 4) order glasses for students based on exam results; 5) provide parent letter explaining eye exam results; and 6) provide referrals for additional follow up where needed at Parker, Markham, Greenleaf, Franklin, Prescott, East Oakland Pride, Brookfield, Westlake, Roosevelt, and Oakland International High Schools, for the period of October 1, 2013 through June 30, 2014, in an amount not to exceed $13,725.00.
