13-2360 Ratification by the Board of Education of a Professional Services Contract between the District and Moira De Nike, San Francisco, CA, for the latter to provide 124 hours of service to develop, with the approval of the Program Specialist, the evaluation design for Tobacco Use Prevention Education (TUPE) Grant; create and distribute project reporting forms for all aspects of the program; devise student survey form for quit and awareness groups and the year-end student survey; input all reporting forms for analysis and send out a monthly report/update to Program Specialist, through the Family, Schools, Community Partnership Department, for the period of August 26, 2013 through June 30, 2014, in an amount not to exceed $8,680.00.
