S.-18 22-2686 Approval by the Board of Education of First Amendment, Community-Based Organization Master Agreement 900322, Procurement Contract No. 21910, by and between the District (Contractor) and County of Alameda, via its Behavioral Health Care Services Agency (ACBH) (County), Oakland, CA, for the latter to provide Counseling Enriched Special Day Class (CESDC) services in accordance with students' Individual Education Programs (IEPs), replacing and superseding Exhibit B as follows: increasing contract maximum by $39,190.00, from $1,205,857.00 to $1,245,047.00; Composite Agency Budget, Exhibit B-2, is amended and replaced by the attached Composite Agency Budget, Exhibit B-2, dated 01/18/2022; Rate Sheet, Exhibit B-3, is amended and replaced by the attached Rate Sheet, Exhibit B-3, dated 01/18/2022; and Exhibit C is replaced and superseded by the attached Exhibit C; all for the period of July 1, 2021 and June 30, 2022. All other terms and conditions of the Agreement remain in full force and effect.
