V1.-3 21-0214 Approval by the Board of Education of Change Order No. 3, Award of Bid and Contract for Construction Services between District and Digital Design Communications, Oakland, CA, for the latter to provide additional construction services to include installation, relocation and replacement of a faulty Beam Detector that was no longer working and needed to be replaced with a newer model, as explained in the attached Summary of Change Order 3 and attached PCO's 3, 4, and 5, for the Fruitvale Elementary School Fire and Intrusion Alarm Project, in the amount of $13,280.37, increasing the Contract not to exceed amount from $908,545.91 to $921,826.28, and extending the Contract term of January 31, 2019 through April 20, 2020 to January 15, 2021, by an additional 270 calendar days, and authorizing the President and Secretary of the Board to sign the Change Order with said Contractor. All other terms and conditions of the Contract remain in full force and effect.

V1.-4 21-0215 Approval by the Board of Education of Change Order No. 3, Between Owner [District] and Contractor for the Sequoia Elementary School Playmatting Project with Sof Surfaces, Inc. ("Contractor"), Petrolia, ON Canada, for the latter to provide additional construction services as follows: the Contractor realized that there were parts missing from the structure and according to B & G it would take a month to receive parts; instead of sending the crew and semi-truck away, the Project Manager asked if they could start work on the smaller structure, which had plans for biding out later in the year, in the amount of $27,631.59; permission granted for new smaller construction project, increasing the Contract not to exceed amount from $88,390.86 to $116,022.45 and extending the contract term from January 9, 2020 through October 9, 2020 to January 7, 2021, by an additional 91 days, and authorizing the President and Secretary of the Board to sign the Change Order for same with said contractor. All other terms and conditions of the Contract remain in full force and effect.

V1.-7 21-0243 Approval by the Board of Education of General Services Agreement between District and Nor-Cal Moving Services, Hayward, CA [Competitively Bid], for the latter to provide the following services to District (collectively, the "Services"): moving services as needed which includes moving furniture and supplies to the District warehouse (off site); relocate 20 offices spaces on site and move them back when construction is complete, as fully described in the bid package for the Agreement and bid documents (including the Bid Form) submitted by Contractor, with all such documents being incorporated by reference into this Agreement for the Fremont High School Library Relocation Project, in the amount of $29,988.00, which includes a contingency of $15,000.00, as the lowest responsive and responsible bidder, and rejecting all other bids, if any, for the term commencing on February 25, 2021, and terminating upon completion of the services but no later than August 4, 2021, (up to sixty (160) days) and authorizing the President and the Secretary of the Board to execute Agreement with said successful bidder.
