P.-2 20-1007 Approval by the Board of Education of a Professional Services Contract between the District and Hazelrigg Claims Management Service, a Third-Party Administrator ("TPA"), Chino Hills, CA, for the provision of Workers' Compensation claims adjusting services to the District, as described in the Contract, incorporated herein by reference as though fully set forth, for the period of July 1, 2020 through June 30, 2023, in a cumulative amount not to exceed $2,090,772.00, for the term, with the service fee not to exceed the stated amount for each year of the term as stated herein: $669,760.00 for the period of July 1, 2020 through June 30, 2021; $696,550.00 for the period of July 1, 2021 through June 30, 2022; $724,412.00, via the Risk Management Department, for the period of July 1, 2022 through June 30, 2023; and with two one-year options for Contract renewal.
