15-1489 Ratification by the Board of Education of Addendum No. 002 to Professional Services Contract between the District and Identity Fraud, Inc., Walnut Creek, CA, for the latter to continue to provide for an additional year its Core Identity Protection Plan related to the compromise of employee records at 1025 Second Avenue, Oakland, CA, for former and active District employees, in an additional amount not to exceed $72,000.00, increasing the Contract not to exceed amount from $72,000.00 to $144,000.00, and extending the Contract period from July 15, 2014 through July 15, 2015 to July 15, 2016. All other terms and conditions of the Contract remain in full force and effect.
