14-1085 Approval by the Board of Education of a Professional Services Contract between the District and Alameda County Health Care Services Agency, San Leandro, CA, for the latter to provide School-Based Health Centers, which are operated by community-based agencies through contracts with Alameda County and provide a range of services, including case management and mental health counseling, physical exams/sports physical, diagnosis and treatment of minor illness, injury, and medical conditions, STD screening and treatment, health education for students and families, youth development programs, professional development for school staff, outreach to youth and their families, community-wide health promotion events and referrals to health and social service providers on and off-site, via FSCP Department, for the period of February 3, 2014 through June 30, 2014, in an amount not to exceed $180,000.00.
